Baseline Sex Differences in Pulmonary Arterial Hypertension Randomized Clinical Trials

Summary: This study found that sex differences have an impact on baseline 6-minute walk distance, hemodynamics, and functional class in pulmonary arterial hypertension (PAH). Females had shorter 6MWD, higher cardiac index, and lower right atrial pressure and mean pulmonary artery pressure. Age and body mass index modulated the relationship between sex and these outcomes. The study suggests that sex, age, and body size should be taken into consideration in the evaluation and interpretation of surrogate outcomes in PAH.

The statistical fragility of the distal fibula fracture literature: A systematic review of randomized controlled trials

Summary: The purpose of this study was to evaluate the statistical fragility in the distal fibular fracture literature using the fragility index (FI) and fragility quotient (FQ). Six randomized controlled trials were analyzed and it was found that the overall FI and FQ were 5 and 0.089, respectively. The study suggests that relying solely on a P value threshold for statistical analysis in the literature may be misleading, and standardized reporting of P value, FI, and FQ can help draw reliable conclusions based on outcome fragility.

Recruitment in Asia and Latin America has contributed to an increase in non-White participants in recent years.

Clinical features of pulmonary arterial hypertension associated with systemic sclerosis

Summary: Systemic sclerosis is an autoimmune disorder that affects the connective tissue and causes skin thickening and complications in the internal organs. Pulmonary involvement, including pulmonary arterial hypertension and interstitial lung disease, is the main cause of morbidity and mortality in scleroderma patients. Current treatment options are not specific to pulmonary involvement in scleroderma, and patients with pulmonary arterial hypertension in this population tend to have worse outcomes and lower response to therapy compared to other forms of Group I pulmonary hypertension.

Pulmonary artery endothelial cellular expression of ST2 suggests that ST2 is a more pulmonary vascular-specific marker for pulmonary hypertension.

The nursing intervention was effective in reducing the occurrence of falls in older adults with arterial hypertension.

Summary: In patients with pulmonary hypertension, exposure to hypoxia led to a shorter constant work-rate exercise test time compared to normoxia, accompanied by hypoxemia, lactacidemia, and hypocapnia. However, pulmonary hemodynamics and dyspnea remained unchanged, indicating that the exercise limitation induced by hypoxia may be due to reduced oxygen delivery causing peripheral tissue hypoxia, increased lactic acid production, and hyperventilation.

The results showed that LUS-derived B-line scores were moderately correlated with LVEDP and PAWP. A B-line index >= 28 can be used to predict elevated LVEDP and PAWP with high specificity.

Implementation of Tele-ICU during the COVID-19 pandemic

Summary: This study describes the implementation of a Tele-ICU program during the COVID-19 pandemic and analyzes the results from the first four months of operation. The program, which aimed to standardize care for COVID-19 patients, was successfully implemented with evidence-based design and remote training for participants. Over 100,000 views on online platforms were recorded, and 326 COVID-19 patients were evaluated, with an ICU mortality rate of 65%. The Tele-ICU program provided multidisciplinary training and clinical follow-up for critically ill patients, demonstrating feasibility and encouraging the development of similar evidence-based projects.

Summary: This study aimed to identify protein pathways associated with delirium post-esophagectomy. The findings revealed that coagulation factor IX and mannosyl-oligosaccharide 1,2-alpha-mannosidase were associated with delirium, while complement C2 and mannosyl-oligosaccharide 1,2-alpha-mannosidase were associated with delirium severity.

Efficacy and safety of fluticasone propionate/salmeterol and fluticasone propionate monotherapy in step-up treatment of childhood asthma: A systematic review and meta-analysis

Summary: Fluticasone propionate and salmeterol (FSC) are viable options for step-up treatment in asthmatic children, as they can reduce the incidence of asthma exacerbations and improve lung function. FSC treatment for more than 12 weeks shows a greater likelihood of reducing asthma exacerbations and decreasing the use of albuterol. There were no significant differences in drug-related adverse events between FSC and fluticasone propionate (FP) alone.

Summary: This study aimed to evaluate the value of LUS score at VA-ECMO withdrawal in neonatal ARDS patients. The results showed that LUS score significantly improved during VA-ECMO treatment and could be used to evaluate various lung diagnostic signs. Additionally, LUS score was positively correlated with ECMO blood flow and could predict ECMO weaning success.

A family intervention to prevent postoperative delirium in patients undergoing cardiac valve surgery: A randomized controlled study

Summary: This study aimed to investigate the effects of family intervention on the incidence of postoperative delirium (POD) and ICU prognoses of patients undergoing cardiac valve surgery. The results showed that family intervention could reduce the incidence of POD, shorten ICU stays, reduce the incidence of anxiety and depression in family caregivers, and improve their satisfaction. Therefore, family intervention is of significant importance for patients undergoing cardiac valve surgery.
